Introduction
1. In order to strengthen the management of retired (resigned) personnel and to protect the legitimate rights and interests of retired personnel， this Certificate hereby is made.
2. As one of the certificates to prove employee’s retirement (resignation)， this Certificate should be filled in accordance with retirement approval sheet by either the employer or the entity that handles the employee’s retirement (resignation). The content hereof should be consistent with original information recorded on employee’s personal file and the official seal should be affixed by issuing entity. Otherwise， it does not take effect.
3. This Certificate should be kept properly and cannot be lent to any others. If it is lost， the holder should inform the issuing entity of its invalidity and conduct re-application procedure.
